Endometriosis and POTS

Pelvic inflammation meets the upright reflex.

The observation, stated once

Orthostatic symptoms are reported frequently by people with pelvic pain, and the two diagnoses co-occur more than chance would predict.

Through the mechanism

Sustained visceral pain raises sympathetic drive continuously. A system already running high has less margin left for the demand of standing up.

Heavy menstrual bleeding depletes iron. Lower blood volume and lower oxygen-carrying capacity both narrow the same margin, which is why ferritin belongs in this workup.

The luteal phase raises resting heart rate and shifts fluid, so orthostatic tolerance is measurably worse in the days before a period — the same days pain peaks.

Mast-cell mediators released in pelvic tissue drop vascular tone directly, joining the two mechanisms at the vessel wall.

What it changes in practice

  • Check ferritin, not just hemoglobin, in anyone with heavy periods and lightheadedness.
  • Do the active stand test in the luteal phase if the follicular one is normal but symptoms are not.
  • Expect autonomic symptoms to improve when bleeding and pain are controlled.

Hold this loosely

Direction of causation is unresolved; both conditions are also underdiagnosed, which inflates apparent association in specialist clinics.
