Long Covid and POTS

The clearest natural experiment we have.

The observation, stated once

A substantial minority of people with Long Covid meet full POTS criteria, and a larger group has orthostatic intolerance without meeting them.

Through the mechanism

A single dated infection followed by autonomic dysfunction is as close to a controlled experiment as this field gets — it establishes that autonomic dysfunction can be acquired, not constitutional.

Small-fiber nerves carry autonomic traffic and are vulnerable to immune-mediated injury. Skin biopsy findings in this group support that route.

Persistent immune activation keeps vascular tone low and heart rate high through mediator effects, independent of nerve injury.

Where post-exertional malaise is also present, the exercise reconditioning that helps other POTS phenotypes causes lasting harm — this is the single most important fork in the pathway.

What it changes in practice

  • Screen for post-exertional malaise before any exercise recommendation, every time.
  • Do a formal active stand test rather than relying on symptom report.
  • Treat this as an acquired autonomic injury with a recovery timeline, not a fixed trait.

Hold this loosely

Recovery trajectories vary widely and predictors are not established; be sceptical of anyone promising a timeline.
